This memorandum of agreement outlines the fee structure for the College in the High School (CIHS) program at the U of M Crookston. * CIHS course should be the same as U of M Crookston course. - Course Request Form
Schools and Instructors wishing to offer a course at their school through the CIHS program can submit a Course Request form for approval. - FERPA Form
FERPA form for student use. Gives a third party (parent) permission to talk about U of M Crookston course and the student. - Human Resource Information Form
This must be completed to allow instructors to submit grades online, check class lists, communicate with other faculty and students, and more. Please complete and return to umccihs@crk.umn.edu. - Syllabus Template
The syllabus is a living document which the instructor has the right to modify and change as the term progresses. Upload to Course Syllabus Management System (CSMS).
